                SUBJECT:          Complaint Investigation Report 

FY2006: No complaints have been received to date.

FY2005: Two (2) complaints were received. Of those, One (1) remains under investigation; and one (1) awaits Board determination.

For Board Determination:

WWP-S3A-1-2005-1                C alleged that R was representing self as the responsible charge operator at a water system using a surface water source. Because the system uses surface water, DEQ has determined that a DWVSWS licensee cannot be the RIC operator. IBOL records confirm that R holds a DWVSWS license.  INV revealed that R was issued a class 1 DWT license in April 2005. Due to R's compliance, IBOL recommends the Board authorize closure.
