Frequently Asked Questions

Q: To qualify for licensure, do I have to complete a residency program?

A:  YES. A minimum of 12 months completed surgical residency is required for licensure.

 

Q:  Is there another examination required for licensure other than the National Board examinations?

A  If you have not held a license in another state for a minimum of the last five years, with the last three of those years actively practicing, you will be required to take Idaho’s State Examination.

 

Q: Where is the State Examination held, and how often?

A:  The State Examination is held at 1109 Main St., Suite 220, Boise, ID 83702 on the third Monday of July each year.

 

Q: Is the PM Lexis examination from the National Board a requirement for licensure?

A:  YES. Passage of all parts of the National Board examination is required for licensure.

 

Q: Is there an application deadline date to qualify to take the July State Examination?

A:  Applications must be submitted to the Bureau a minimum of 90 days before the examination date.

 

Q: I have let my license lapse, what must I do to activate that license?

A:  A license that has been lapsed less than five years may be reinstated upon submission of a renewal application together with the payment of all back renewal fees, a license re-instatement fee, and proof of completion of 12 hours of continuing education for each year the license was lapsed.  If a license has been lapsed for more than five years, a new application for licensure is required. Please contact the Bureau.

 

Q: I am applying for licensure in another state and they require verification of my license in Idaho. What do I need to do?

A:  Send a written request to:  The Bureau of Occupational Licenses, 1109 Main St., Suite 220 Boise, ID 83702 along with a $10.00 processing fee, and the address of the office that requires the verification. We will send the verification directly to that state.
