Reinstatement Fee.................$250

All Acupuncture licenses expire before the licensee's birth date each year and must be renewed annually before that date on forms provided by the Board. Licenses not so renewed will be cancelled.  Any license cancelled for failure to renew may be reinstated any time within 5 years upon payment of a $250 renewal application fee accompanied by a renewal application, proof of meeting the continuing education requirement, and any back years license renewal fees.  

All fees are non-refundable.
